All participants who wish to participate in activities on the premises must read, agree to, and complete this document prior to commencement of any activity.
In checking the box below, I agree that Iron Sharpens Iron Brazilian Jiu Jitsu Academy (ISI BJJ) is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at may be physically strenuous, and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
I understand that I am participating in a sport that has physical contact. I assume full responsibility for all of my actions during and connected to my training at the Iron Sharpens Iron Brazilian Jiu Jitsu Academy (ISI BJJ). I understand the risk of taking part in this type of sport. Therefore, I hereby release, waive, discharge, ISI BJJ and all persons associated with ISI BJJ, including but not limited to, it's owners, officers, agents, employees and volunteers, from any and all liability claims demands actions, causes of action whatsoever arising out of or related to an loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon the premises where the activity or contact is being conducted. I, the undersigned also state that to the best of my knowledge, I am in good health and physical condition and know of no reason why I cannot participate in this training.I hereby further represent that I have no medical or other condition, that would expose myself or others at ISI BJJ, to any type of risk whilst participating in classes, instruction or use of the facilities at ISI BJJ.
COVID-19 Contact Tracing
We’re collecting personal data for the purpose of carrying out contact tracing and other measures in response to any emergency that may arise due to the novel Coronavirus (COVID-19) that may threaten the life, health, or safety of our members and any visitors, their family and friends and in particular, any vulnerable persons that may be exposed to them.
The information will be destroyed once there are no business/legal purposes to have them on record. ISI BJJ will decide on access to our premises based on the answers provided. We have the right to refuse entry to any persons who are unwell, who have been to locations of public health concern in the past 14 days or who have been in contact with any person with a confirmed or suspected case of the coronavirus.
You are not permitted to attend our premises if you currently have flu or cold like symptoms. In addition, in order to have an effective contact tracing and response plan in place, it is very important that should you develop any cold or flu like symptoms whilst you’re frequenting ISI BJJ and/or after you sign this declaration that you self-isolate immediately, and inform us, and all persons you have come into contact with. In addition, should you travel at a future date you must notify us before returning to the academy.
By checking the box below you also certifying that:

You have NOT been in contact with any person who is a confirmed/suspected case of COVID-19 in the past 14 days.

You DO NOT currently have a fever or any flu like symptoms (including but not restricted to, a persistent cough, runny nose, sore throat and shortness of breath), and

You have NOT you travelled outside the UK in the last 14 days.
A temperature check may be administered by ISI BJJ staff when you enter the premises and if a fever of 38C or higher is detected, ISI BJJ reserves the right to request that you leave the premises. By checking the box below you are consenting both to this check and also that you will comply, and co-operate with any instructions issued by ISI BJJ as part of our coronavirus health & safety precautions.
I further certify I am at least 18 years of age. If under 18 my parent/guardian is the below signed. In case of emergency, I hereby authorise any licensed medical personnel to perform any accepted medical procedure deemed necessary and I agree to bear any potential expenses of any such treatment. I also agree that my attendance and/or performance at training may be photographed, filmed, or taped and used for marketing purposes by any schools and I waive any compensation thereof.
